Quick answer: Growth hormone peptides such as tesamorelin, CJC-1295 (no DAC), and ipamorelin work by signaling your pituitary to release its own growth hormone in natural bursts, while your body’s feedback brakes stay on. Injected growth hormone bypasses those brakes. Human studies show growth hormone supports tendon repair and visceral fat loss, and working with a licensed clinician is the best way to use these peptides well.
Growth hormone peptides have become one of the most talked-about tools in recovery and body composition. To understand why people choose them over injected growth hormone, it helps to understand one idea: your body does not release growth hormone as a steady drip. It releases it in pulses.

How Your Body Releases Growth Hormone
Growth hormone (GH) comes from the pituitary gland in short, high bursts separated by quiet periods. The biggest natural burst usually happens in the first hours of deep sleep. Two signals control it: GHRH, the brain’s “release” signal, and ghrelin-pathway signals that amplify each pulse. Somatostatin acts as the brake.
There is also a built-in feedback loop. When GH rises, the liver makes IGF-1. When IGF-1 gets high enough, it tells the brain to ease off. This loop is part of what keeps GH levels in a normal range (Sigalos & Pastuszak, 2018).
What Growth Hormone Actually Does
Fat loss: a direct effect of GH
GH’s most prominent metabolic effect is a marked increase in lipolysis, the release of stored fat for fuel (Møller & Jørgensen, 2009). In a meta-analysis of obese adults, GH treatment reduced visceral (belly) fat and added about 1.8 kg of lean mass, even without changing body weight (Mekala & Tritos, 2009).
Repair: largely through IGF-1
Many of GH’s repair and growth effects run through IGF-1, made both in the liver and locally in muscle and tendon. In a randomized trial in healthy men, two weeks of GH raised local IGF-1 and increased collagen production in tendon and muscle, the scaffold tissue that holds repairs together, without increasing muscle-building protein synthesis (Doessing et al., 2010).
In men aged 65 to 75, GH during two weeks of leg immobilization and six weeks of retraining protected muscle size and produced larger, stiffer tendons than placebo (Boesen et al., 2014). Both studies used injected GH and were small, so they show what GH can do rather than what any specific peptide will do.
Growth Hormone Peptides vs. Injected Growth Hormone
This is where peptides come in. A 2018 review of GH secretagogues concluded that they promote pulsatile GH release that stays subject to negative feedback, which may help prevent the supra-physiological GH levels and side effects linked to injected GH (Sigalos & Pastuszak, 2018). The same review was clear that few long-term, rigorously controlled studies exist.
- Tesamorelin is a GHRH analogue. It is the best-studied option and is approved (as Egrifta) for a specific use described below.
- CJC-1295 without DAC (also called Mod GRF 1-29) is a short-acting GHRH analogue that supports a natural-style pulse. The version with DAC binds to blood proteins and keeps GH elevated for days, which is closer to a constant signal. Read more in our CJC-1295 / Ipamorelin research guide.
- Ipamorelin works on the ghrelin receptor to amplify the pulse and is often paired with CJC-1295.
- Injected GH (somatropin) delivers the hormone directly. It works, but it bypasses the brain’s release signal and much of the feedback loop.
The same logic applies to hormones in general. We explain the broader difference between signaling compounds and direct hormone replacement in Peptides vs. Steroids: What’s Actually the Difference.
Tesamorelin: The Approved Example
Tesamorelin was approved in 2010 to reduce excess abdominal fat in adults with HIV-associated lipodystrophy. A 2026 meta-analysis of five randomized trials found it reduced visceral fat, trunk fat, liver fat, and waist size, and increased lean mass (Badran et al., 2026).
Two details matter. The approval covers that specific population, not general weight loss. And the label notes that tesamorelin is weight-neutral and that long-term cardiovascular safety has not been established. Liver fat reduction has been studied but is not an approved use.
Using Them Wisely: What to Track
- Joint comfort. Raised GH can bring joint aches, fluid retention, and tingling, so tracking how you feel helps your clinician fine-tune the plan. In the obesity meta-analysis, joint pain was about six times more likely than with placebo (Mekala & Tritos, 2009).
- Blood sugar. GH counters insulin, so checking fasting glucose keeps you on track (Møller & Jørgensen, 2009; Sigalos & Pastuszak, 2018).
- Growing long-term research. Tesamorelin has the most long-term data so far, and larger long-term trials of CJC-1295 and ipamorelin in healthy adults are the next step.
- Quality and sourcing. Choosing a trusted, tested source matters. Learn how to judge claims in How to Tell If a Peptide Claim Is Real Science or Marketing.
Anyone considering a GH-raising compound should work with a licensed clinician and track labs, including IGF-1 and fasting glucose, before and during use.
Timing and the Natural Pulse
Because food, especially carbohydrates and fat, blunts GH release, GH-releasing peptides are commonly taken on an empty stomach, often before bed so the pulse lines up with the body’s natural deep-sleep burst. Sleep itself is still the most reliable GH signal you have, which is why deep, consistent sleep matters as much as anything you take.

Frequently Asked Questions
Do growth hormone peptides work?
They raise the body’s own GH release. Tesamorelin has approved evidence for reducing visceral fat in a specific population. For CJC-1295 and ipamorelin, human data show GH increases, but long-term outcome trials in healthy adults are lacking.
Are peptides safer than injected growth hormone?
Peptides keep GH release pulsatile and under the body’s feedback control, which is a logical safety advantage. However, long-term safety has not been proven in large trials, so this remains a reasoned rationale rather than an established fact.
What is the difference between CJC-1295 with DAC and without DAC?
The version without DAC is short-acting and supports a natural pulse. The DAC version binds to blood proteins and keeps GH and IGF-1 elevated for days, producing a more constant signal.
Does growth hormone help with healing?
In small randomized trials, GH increased collagen production in tendon and muscle and improved tendon size and stiffness during rehabilitation in older men. These studies used injected GH, so effects from peptides are likely smaller.
